Patient Positioning
- Patient seated or supine, knee flexed.
- Sole of the foot flat on the IR initially.
- Rotate the foot medially 30° to 40°.
Central Ray
Perpendicular to the IR, directed to the base of the third metatarsal.
Diagnostic Importance: Lisfranc Injury
- Visualization without overlap of the joints.
- Alignment assessment between metatarsals and tarsals.
- Detection of subluxations or fracture-dislocations.
- Joint space evaluation in the medial region.
Lisfranc injuries are hard to diagnose; this oblique view is essential for their evaluation.
